About Hope Medical Clinic
Hope Medical Clinic’s health care professionals provide expert and affordable medical care to keep your entire family healthy, from infants to the elderly. Our clinic offers family practice care to people with and without insurance and to those who are under-insured. Our board certified health care professionals are here for your acute and chronic medical needs from sore throats, earaches and rashes to diabetes and high blood pressure. Hope Medical Clinic also provides well child exams, Medicaid EPSDT screenings, school physicals, and we manage common pediatric conditions. We believe prevention is key to optimal health and we provide yearly physicals, pap smears and preventative screenings. You can receive all the care you need to keep your whole family healthy at Hope Medical Clinic!

Nichole Clark, APRN
Nichole earned her nursing degree and Masters of Science in Nursing while specializing in Family Practice from Vanderbilt University, Summa Cum Laude (4.0 GPA). She was awarded highest honors by receiving the Outstanding Family Nurse Practitioner student of the year award upon graduation from Vanderbilt University. She enjoys spending time with her husband and two beautiful girls, Anniston Rose & Carolina Lily!

Dana Treat, APRN
She earned her Bachelors of Science in Nursing from Baylor University then completed specialty training as a neonatal ICU clinician through the University of Texas at Arlington. After years of rewarding pediatric practice, initially in the NICU and later in school health, Dana returned to school at UAMS where she earned her Master’s in Nursing Science. She loves photography, hiking, writing and travel. You can see her photography on display at the clinic.

Jordan Kendrick, APRN
She is originally from Hoxie, AR and attended the University of Central Arkansas for both undergraduate and graduate degrees in nursing. She has certifications as both an Adult and Family Nurse Practitioner. Jordan loves spending time with family and friends, vacationing at the beach (when she gets the opportunity) and enjoying good food wherever she goes.

Dr. Richard Melin
Dr. Richard Melin was born and raised in a farming community in Western Minnesota. He has been wonderfully married to Sally (former RN) for almost 46 years. They have one son, Paul and were also foster parents for a number of years. They are Spiritually-minded individuals having been involved in many activities, including medical and non-medical mission trips in the U.S. and around the world. He is also an Army veteran.

Good Faith Estimates for Patients Who Don’t Have Insurance
You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost
Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
- You have the right to receive a Good Faith Estimate for the total
expected cost of any non-emergency items or services. This includes
related costs like medical tests, prescription drugs, equipment, and
hospital fees. - Make sure your health care provider gives you a Good Faith Estimate
in writing at least 1 business day before your medical service or item.
You can also ask your health care provider, and any other provider you
choose, for a Good Faith Estimate before you schedule an item or
service. - If you receive a bill that is at least $400 more than your Good Faith
Estimate, you can dispute the bill. - Make sure to save a copy or picture of your Good Faith Estimate.

No Show Policy
Patient may be terminated from Hope Medical Clinic after the 2nd time they do not keep their appointment or do not cancel 24hrs prior to their scheduled visit.
Unfortunately, we have had to implement the above policy due to the increasing number of no show appointments. No show appointments are a detriment to our patients and practice. By not canceling your appointment, this prevents other patients from being seen.
